Enrolment Form

Location - Stratford Dolphins Football Club on the corner of magazine and Arnold street. Classes run identically with the school terms with each enrolment expiring at the end of the school term. All classes will run on public holidays and pupil free days.

TERM 1 - 10 weeks

MONDAY 28 JANUARY - FRIDAY 5 APRIL 2019 

Wet weather- If the outside area of our training facility is deemed unfit for training all classes will be run at the same location but in the club house and outdoor undercover area until outdoor area is deemed safe for training.

Class times and days may change and extra classes may be established prior to the school term starting due to not enough or to many enrolments for a certain age group.

To finalise your child's enrolment in NGactive Classes, please complete and submit the following information. Once you have completed this form, you will be sent an invoice in next 24hr to which you have 7 days to complete payment and secure you child's/children's position.

Note 25%discount will apply to second and third child.

There are limited positions per class in all age groups so it is very important to complete payment to complete your registration and secure your child's/children's spot.

TRAINING KITS

Training Kits are available and contain the following items; training top, shorts,drawstring bag, Gym towel, water bottle and Hat (cost $99) with the option of name on the bag and training top.

On completion of payment you will be contacted to confirm your child's sizing prior to orders being placed.

By submitting this form you agree to the following: I agree to let my child/children participate in the NGactive program. I understand that there are certain risks of injury in the participation of this activity and personal property may be lost or damaged and I am willing to assume these risks. I confirm that my child/children are capable of participating in our fitness program and that my child/children are in good physical condition. In addition to giving full consent to my child's/children's participation I release, indemnify and hold harmless the NGactive instructor, its servants and agents, from and against all and any actions or claims which may be made by me or on my or my child's behalf or by other parties for or in respect of or arising out of any injury, loss, damage or death caused to me or my property whether by negligence, breach of contract or in any way whatsoever. I grant permission for my child/children to receive emergency medical treatment. I grant NGactive permission to use photographic images of my child/children in its promotional activities.
